A 60-year-old man complains of pain in his shoulder
that has persisted for three months. There is no history
of trauma. What is your impression of this frontal film
of his shoulder?

ANSWER
The correct diagnosis is malignancy. The spotted lucency
of the bone and the presence of a pathologic fracture
point to a malignant bone lesion. It would be impossible
to determine whether this is a primary lesion or a metastatic
deposit based on this film alone. Biopsy proved it to
be a chondrosarcoma.
