This 20-year-old man injured his wrist in a soccer
game. What is your interpretation of the frontal and
lateral projections of his wrist?

ANSWER
The correct diagnosis is Madelung's deformity. This
deformity may be part of a larger skeletal dysplasia,
but it may also occur as a unilateral isolated event
in otherwise normal individuals. The chief findings
are that the carpal angle is increased and the distal
end of the ulna is in a dorsal position. Impaired development
of the ulnar aspect of the radial epiphysis is believed
to be the underlying cause.
