PROBLEM
A 90-year-old man complains of abdominal distension and left groin pain. What is your impression of this film of his pelvis?

ANSWER
The diagnosis is incarcerated left inguinal hernia, evidenced by the gas collection visible near the left ischiopubic junction. The
patient’s abdominal distension probably indicated early small bowel obstruction.
