ANSWER

The fractures of the pubes and ischia are obvious. However, note the accompanying injuries in the posterior portion of the pelvis, including a fracture of the right side of the sacrum and a dislocation of the left sacroiliac joint. Anterior ring pelvic fractures are often associated with companion fractures of the posterior pelvic ring as in this case. Careful search should be made for these routinely.

This series of diagnostic quizzes that challenge your ability to read a variety of x-ray films is edited by Dr. Keats, professor of radiology and professor of orthopedics at the University of Virginia School of Medicine in Charlottesville.
