This two-day-old boy is brought to the emergency department
in respiratory distress. What is your interpretation
of his chest film?

ANSWER
The diagnosis is left diaphragmatic hernia. Multiple
loops of air-filled bowel are evident in the left hemithorax,
with resulting displacement of the mediastinal structures
to the right. This condition is often associated with
hypoplasia of the underlying lung.
